What you are buying
You are buying a licence to use a digital file, not its copyright. Everything sold here remains the intellectual property of Nate Products.
What you may do
- Use it for your own household or family.
- Fill it in, print it as many times as you like, and keep printed copies wherever they are useful.
- Share a filled-in copy with the people involved in the situation it covers.
What you may not do
- Resell, sublicense or redistribute the blank file, in whole or in part.
- Republish it, place it behind another paywall, or include it in another product or bundle.
- Use it as the basis of a competing product, or to train a machine learning model.
- Use it commercially, for clients, patients or an organization’s staff, without asking first. Ask; the answer is often yes.
Nature of the products
The products sold here are organizational tools. They are not medical, legal, financial or insurance advice. They are not healthcare software or a medical treatment, and they are not official or hospital-approved documents. No organization is obliged to accept or act on them, and they do not replace advice from your own professionals or the records those professionals hold.
Information you enter is only as accurate as the day you entered it. Review it regularly.
The products are provided as they are, without warranty of any kind. To the extent the law allows, liability for any claim connected to a product is limited to the amount you paid for it.
Payment, delivery and refunds
Orders are processed by Lemon Squeezy as merchant of record, who issue your receipt and handle sales tax where it applies. Delivery is by email, normally within a minute of payment. Refunds are covered on therefunds page, which forms part of these terms.
Changes
