What this is
When someone in a family starts needing care, the hard part is rarely the care itself. It is the coordination around it: remembering which pharmacy, knowing who already called the doctor, finding the insurance card at nine at night, and repeating the same medication list to the fourth person who asks.
Most of that information already exists. It is spread across one person’s memory, three phones and a drawer, which works until the person holding it is the one sitting in the waiting room.
The Family Care Coordination Kit is a place to write it down once, together, on a calm day, and to hand to whoever needs it next. It is deliberately small: six sections, no filler, and nothing that duplicates what your clinicians, insurer or lawyer already hold.
Who it is for
- Families coordinating care for an aging or unwell relative
- The person who became the default organiser and would rather not be the only one who knows everything
- Households where more than one person needs to be able to act
- Anyone who has sat in a waiting room trying to remember a medication dose
Who it is not for
- People looking for medical, legal or insurance advice. This is an organizing tool, and no facility is required to accept or use it.
- Anyone wanting somewhere to store account numbers, passwords or PINs. The kit records where those are kept, not what they are.
- Readers outside the US and Canada who need local emergency numbers and coverage examples built in. The structure works anywhere, but those details need adjusting.
What is included
6 sections across 11 pages, plus a cover record and a review log.
Section 01: Quick-Start & Sharing Instructions
The order to fill it in, how to keep it current, and how to share it without losing track of who holds a copy.
Section 02: Family Role Map & Contact Tree
Who is responsible for what, who calls whom, and who speaks about healthcare decisions. Agreed before a crisis, not during one.
Section 03: Emergency Contacts
The short list: people to call first, care and support contacts, and the details you would otherwise be searching for on a phone.
Section 04: Medical Snapshot
Allergies, ongoing conditions, current medications and the care team. The essentials you may be asked for on arrival.
Section 05: Insurance & Key-Document Locator
Coverage details and where the originals are kept. A map, not a vault: it records where things live, never account numbers or passwords.
Section 06: Hospital / ER Go-Bag Checklist
What to pack now, what to bring, and what to write down while you are there.
Also inside: cover and kit record (page 1) and about this kit and review log (page 11).
Real pages from the kit.
4 of 11 pages shown, plus a detail from page 7. The rest stay in the file.

How it works

Fill it in once, on a calm day
About twenty minutes. Start with Emergency Contacts: if you stop after one page, make it that one.
Share it with the right people
Give access to the smallest useful group, the people who would actually act. The Google Doc is the living copy; the PDF is the printed snapshot.
Keep it current
Review it every few months and after any hospital stay, medication change or move. Write the review date on the cover.

Questions
All questionsDo I type into it, or print it?
Both. Every field is fillable on a computer, and it prints cleanly on US Letter. Most people keep the PDF as the working copy and print Sections 3, 4 and 5 for the go-bag.
What is the Google Doc for?
It is the living version: easy to edit and to share with named people. The PDF is the printed snapshot. The kit suggests updating the doc first, then reprinting.
What exactly do I get, and how quickly?
The 11-page fillable PDF and a companion Google Doc. A download link arrives by email within a minute of paying.
Is this medical or legal advice?
No. It is a personal organizing tool. It does not replace advice from your clinicians, or the official records your providers and insurer already hold.
Will a hospital accept it?
Nothing in it is required or guaranteed to be accepted by any facility. It exists so the information is ready when someone asks for it.
Should I put passwords or account numbers in it?
No, and the kit is built to stop you. It records the last four digits at most and notes where the full details are stored.
Does it work outside the US and Canada?
The structure does. The emergency numbers and insurance examples are US and Canada oriented, and the kit asks you to confirm the details that apply where you live.
How long does it take to fill in?
About twenty minutes for the first pass. After that, a short review every few months and an update after any change.
